To establish a fraud alert on your AcuFi customer file, complete this form.  To prevent the unauthorized use of your information, messages are sent to potential credit grantors to alert them to verify your identity before granting credit in your name.  This alert will remain on your file for 90 days and will automatically expire unless you remove it before the period concludes.  In order to submit a request, all of the fields listed below must be completed.  AcuFi will exclusively utilize your information to verify your identity and process your request.  We may be unable to fulfill your request if we are unable to verify your identity or connect the material you provide in your request to personal information we already have.  In the event that we encounter difficulty in fulfilling your request, we will send you an email to notify you.
