To obtain a copy of your AcuFi Consumer File, please complete the form provided below. If you require access to the personal information associated with any AcuFi services, please utilize the AcuFi Privacy Request Form instead.
Each of the subsequent fields must be completed in order to submit a request. AcuFi will exclusively utilize the information you provide to verify your identity and process your request. We may be unable to fulfill your request if we are unable to verify your identity or link the information you provide in your request with personal data we already have. In the event that we are unable to accommodate your request, we will notify you via email.
